The Little Tennessee Valley Educational Cooperative (LTVEC) will operate and maintain the statewide equipment lending library through the Tennessee Assistive Technology Project (AT Project).
The borrower (an authorized school district employee or contract clinician) is responsible for the condition of hardware and software that they borrow from the AT Project. If items are not returned, the school district will be charged a replacement fee for each item not returned, plus shipping costs.
- Loans will only be shipped to a School District Central Office.
- The Special Education Supervisor must have an MOU on file with AT Project
- AT Project pays shipping cost on check out.
- School Districts are responsible for return shipping costs.
- Loans are for a 30 day trial period. Five (5) items can be requested/ reserved at one time, with the exception of two (2) iPads, due to high demand.
Loan descriptions list the included contents. In the event of missing, damaged, or malfunctioning items or parts, you should contact AT Project staff immediately for further instructions.
Borrowers are responsible for confirming the condition of library items they receive and immediately notify AT Project staff of any missing, damaged, or malfunctioning items. Borrowers assume full responsibility for issues not reported within the following time period:
- Pick-up/Delivery: three days from the check-out date. (ONLY Applies to School Districts working with AT Project Staff as a Pilot district OR contracting with LTVEC for services).
- If items are not returned, the school district will be charged a replacement fee for each item not returned, plus shipping costs.
- Mail/shipping: seven days from the check-out date.
In the event of lost, damaged, or unreturned items and/or parts, the primary contact person on the borrower account will be responsible for paying any replacement/repair costs as necessary to restore the item to its full functioning condition.
The borrower may request to pay or resolve the cost in lieu of billing the school district.
The prices of items are subject to change; replacement costs might be different than the item price listed in our library database MyTurn.
(Our "Replacement Cost" factored in the cost of the item and the shipping costs when we purchased the item. The district will be charged the price of the item and shipping cost to replace that item.)
If the library item(s) are 2 months or more overdue/unresolved, the school district will not be able to borrow items until the items are returned or replacement cost is paid.
Loan Liability Agreements - I agree to the following statements:
- I have read, understand, and agree to abide by the Loan Liability Policies.
- I will use this device only for the specified purpose.
- I agree to pay/resolve replacement/repair costs to the AT Project for any lost, damaged, or unreturned items and/or parts.
- I understand and accept that if the borrowed library item(s) are greater than 2 months overdue/unresolved, AT Project staff will send an invoice to the school district or organization, according to the library policies schedule for damaged/lost and overdue fees.
